Terms and Conditions for Live Well Membership Plans
Live Well Memberships are on a monthly basis and require a minimum of a 3-month commitment. Your membership will renew on the date you signed up and will recur on the same day each month thereafter until your membership is terminated. Membership fees will be charged to your credit card on file.
In accordance with this agreement you are required to provide a 30-day written notice activating termination of your membership. All unused sessions will expire 30-days after termination of your Membership agreement. No additional charges will be posted after the 30-day termination notice.
Membership is not transferable. Member pricing is not good with any other promotional offer and may not be combined with any other offers or discounts
